When you click the MySQL button
for the first time, simply click the "Add" button.
Your database will now be created.

MPORTANT!
The password that is in
place at the time of the installation is what
MySQL will use for the database. If you change
your Control Panel password later, it will
*NOT* change the MySQL database password. Keep
this in mind when installing MySQL for the
first time!
